Plot
The game takes place outside North America, eleven months after the formation of the UCA. Sam Bridges, now living a quiet life with Lou, finds himself in Mexico following a request from Fragile. In Mexico, Sam gets caught up in an incident that eventually brings him to Australia. There he must take on the herculean task of traversing across the continent to, once again, re-connect a fractured society, expanding the Chiral Network as part of a private organization.
Gameplay
The game features real-time terrain change like floods, forest fires, and earthquakes.
Structures can be damaged and destroyed by the real-time terrain change.
Sam can now put his backpack on the floor, which start floating like a carrier.
The game features a Photo Shoot event where the player takes pictures that will be saved as a Polaroid, which can be viewed later, and the photos have influence on the game’s story.
The game has an "easy mode" that allows players to skip boss battles. This is for players who aren't familiar with action games and/or are just interested in the story.
The game features some new weapons like the Electric Rod (the MULE’s weapon of choice) can be used to strike or be thrown in this game to electrocute vehicles and enemies and various holographic grenades like a BT holographic grenade that projects a hologram of a BT when activated, causing human enemies to flee.
New players can catch up on the story of the previous game via ‘Death Stranding Recap’ from the title menu and look up game terminology from the in-game ‘Corpus’ feature.
The game features an option to switch to an ‘aerial camera’ that captures Sam from an elevated bird’s-eye view, which allows the player to take in the vastness of the environment as Sam slowly makes his way across the terrain.
The game features real-time day/night cycle which the player can use to their advantage.
Porter Grade has been replaced with 12 new ‘proficiency’ levels. Using a weapon more than another tool, or walking more on rough terrain with unstable footholds rather than using a vehicle, directly impact Sam’s stats and abilities.
The game features the Automated Porter Assistant System Enhancement, divided into four different traits (porter, combat, stealth, and servicemanship), a component that modifies Sam’s abilities by consuming ‘memories’ acquired during the game.
The game features new enemies: a new type of BT called the ‘watcher’, which detects and attacks Sam through sight, and a new human antagonist, including a group called ‘Armed Survivalists’ who assault with lethal firearms.
The game features a new infrastructure for traversing: the Monorail.
